License/Exam Fees:

$100 Non-Refundable Application Fee
$200 License Fee -- two year license

Licensing Requirements:

Applicant must meet at least two of five minimum requirements:

completed at least 350 hours at a state approved massage training school or program
hold current professional class membership in a recognized national massage organization
hold current certification by the National Certification Board for Therapeutic Massage and Bodywork or a similar organization
have been licensed out of state within the past three years of application
have had two years' experience within the last five years prior to application

Applicants must also be at least 18 years of age, not be addicted to drugs or narcotics, have no misdemeanor convictions for assault or dishonesty within three years of application, no felony convictions within five years, and never had any sexual misconduct convictions.

Along with the application, applicants must submit copies of required certificates, drivers license, Alaska Business License and their Alaska criminal background report.

Applicants must provide 5 years of criminal history. The applicant will need to provide a criminal history from each state of residency during the above-mentioned time period. The Alaska report must be dated the same date that the application is submitted.
