General dentist:
- Application by exam fee $300

Dental specialist:

Licenses are issued biannually and all current licenses will expire December 31, 2012
Licensing Requirements:
Applicants must have graduated from an American Dental Association accredited school of dentistry. Dental specialists do not need a general dentist license. Must provide documentation acceptable to board that shows applicant has completed as many academic years of advanced education in the specialty as are required at the time of application by the appropriate specialty board that is accredited by the Commission on Dental Accreditation of the American Dental Association. Documentation acceptable to the board that shows that the applicant is a diplomate or the equivalent of the appropriate specialty board or is eligible to be examined for diplomate status as documented by an organization recognized by the American Dental Assoc. Pass Alaska state Jurisprudence Exam.
